Starting on May 1, 2022, I will only be providing tele-health or virtual therapy only. Services may be covered in full or in part by your health insurance or employee benefit plan. It is important to verify whether tele-health services are covered. Please check your coverage prior to the appointment carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
I am currently in network with the following insurance companies:
Blue Cross Blue Shield
United Behavioral Health/Optum Behavioral Health
I also accept self pay. Please note, many insurance companies provide OUT OF NETWORK benefits as well. If I am not in network with your insurance provider, please inquire about those benefits.
Cash, check and all major credit cards accepted for payment. Payments, including co-pays or co-insurance, are due in full at the time of the appointment. There is a $25 fee for returned checks.
If you miss your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay $75 for a missed session. This fee is not covered by your insurance carrier. All cancellations should be made by calling or texting my office at 847-892-6390.
