Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage prior to the appointment carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
I am currently in network with the following insurance companies:
Blue Cross Blue Shield
United Behavioral Health/Optum Behavioral Health
I also accept self pay. Please note, many insurance companies provide OUT OF NETWORK benefits as well. If I am not in network with your insurance provider, please inquire about those benefits.
Cash, check and all major credit cards accepted for payment. Payments, including co-pays or co-insurance, are due in full at the time of the appointment. There is a $25 fee for returned checks.
If you miss your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay $75 for a missed session. This fee is not covered by your insurance carrier. All cancellations should be made by calling my office at 847-892-6390.
